Qoder ignored the carefully crafted design document in quest mode
Steps to Reproduce
Start quest mode
Chat in multiple steps, ask to change or remove features, updating the document
Click start tasks
Expected Behavior
Task list will be generated to review
Actual Behavior
Qoder generated 6 to-dos covering THE LAST CHAT MESSAGE only, completely ignored the carefully crafted design
No options to revert, retry, continue
